From my experience playing the PC version, a gbA+ or gbB+ only results in a hit with the infield in. Otherwise the + is ignored. I don't recall it having anything to do with runners being held.

Even with the pc version or what strat prefers to call the cdrom version how this works depends on what settings are used.
That is part of the confusion here. For a long time strat would not allow TSN to share what settings were turned on and what was turned off. Little by little things have been leaked/shared. But there is still some contradictions between what we have been told and what is on the online strat pages.
